Clinical Policies

1. Preceptor Requirements

Preceptors may be any healthcare provider (MD, DO, APRN, PA excluding CRNA) who has been practicing in the field for at least one year and has a current, unencumbered professional license.  Preceptors will be oriented to the student learning outcomes and YSN clinical policies through a preceptor orientation packet that will be distributed by the student.  Students must spend a minimum of 50 hours with each individual preceptor.  Each student may have a maximum of six preceptors.

Division of Responsibilities

Student Responsibilities

  1. Collaborate with Clinical Coordinator to secure clinical rotations.

  2. Complete necessary documentation through Castlebranch and Typhon prior to clinical rotations.

  3. Contact clinical sites to determine specific clinical days and complete any prerequisite training at their site.  All prerequisite training must be completed prior to the first day of the clinical rotation.

*Failure to contact clinical sites may result in loss of the clinical placement.  Students are responsible to find replacement clinical rotations in this situation.

  1. Adhere to professional attire and behavior in the clinical setting at all times.

  2. Collect and enter all patient encounter information in Typhon within 10 days of the clinical experience.  If not entered within 10 days, the clinical hours for that day will not count and will need to be made up.  Exceptions may be considered by clinical faculty on an individual basis.

  3. Demonstrate increasing competency in assessment, management, and presentation of patients.

  4. Function in the role of the nurse practitioner under the supervision of the clinical preceptor incorporating evidence-based practice guidelines.

  5. Attend all scheduled clinical experiences on time and be prepared, completing all required clinical hours by the final date of each clinical course.

  6. Complete clinical preceptor and clinical site evaluations at the end of each clinical rotation.

Preceptor Responsibilities

  1. Complete the Preceptor Agreement Form and facilitate completion of the affiliation agreement if necessary. (Appendix A)

  2. Review Preceptor Orientation Packet which will be provided to the preceptor prior to clinical rotation.

  3. Orient the student to the clinical site and agency policies.

  4. Discuss with the student the preferred method of communication with the preceptor/clinical site.

  5. Discuss preceptor and agency expectations for the documentation of patient encounters.

  6. Facilitate an informal collaborative and mutually respectful environment in which to learn.

  7. Review the student’s clinical learning outcomes to determine the types of experiences that will enhance the students learning.  Direct the student to resources and evidence-based readings.

  8. Promptly communicate to KCU faculty regarding issues of concern or unsafe practice.

  9. Facilitate students’ progressive independence in clinical knowledge and skills by supporting the student’s autonomous assessment and evaluation.

  10. Review and sign the student’s initial self-evaluation form and complete the final clinical evaluation.

Faculty Responsibilities

  1. Validate current unencumbered licensure of all preceptors prior to the start of the clinical rotation.

  2. Communicate with clinical preceptors to discuss the student’s role in the clinical rotation and ensure that the student is working toward meeting learning outcomes.

  3. Assist student and clinical preceptor to optimize clinical learning environment.

  4. Review Typhon clinical electronic log entries each week throughout the semester.

  5. Provide preferred methods of communication and be available to answer questions or concerns regarding the student’s clinical experience.

  6. Award student final grade upon achievement of clinical competencies and in consultation with preceptors.

  7. Review the student’s evaluation of the clinical site and preceptors and provide information to the faculty course coordinator.

  8. Provide written documentation to the preceptor of the semester, year, course, and hours worked with the NP student at the end of the rotation.

2. Student Health Requirements

Documentation of compliance with clinical health requirements must be verified prior to students participating in scheduled clinical experiences.  It is the student’s responsibility to ensure compliance with any and all clinical facility requirements. Failure to comply with CDC or facility requirements may result in inability to attend clinical rotations, resulting in course failure.  Each student must have documentation of the following requirement submitted and approved no later than one month prior to the beginning of clinical rotations.

  • Current Health Insurance: Students must provide documentation of current health insurance.

  • Current Health Screening: Students must provide documentation of current health screening by licensed healthcare provider and certification for clinical practice (KCU Health Form).

  • Current Immunizations: Students must provide documentation of immunizations and TB screening in compliance with the requirements established according to the current Centers for Disease Control and Prevention (CDC) guidelines that can be found at http://www.cdc.gov/vaccines/. Clinical agencies may have additional requirements.

  • TB Screening: There must be documentation of the following:

    • Annual TB screening assessment
    • TB blood test or initial 2-step TB skin test initially; TB test annually thereafter
    • If positive results, provide a clear Chest X-Ray (lab report required)
  • Measles, Mumps, & Rubella (MMR): There must be documentation of one of the following:

    • Two vaccinations
    • Positive antibody titers for all 3 components (lab reports required)
  • Varicella (Chicken Pox): There must be documentation of one of the following:

    • Two vaccinations
    • Positive antibody titer (lab report required)
    • Documented history of disease (dates required)
  • Hepatitis B: There must be documentation of one of the following:

    • 3-dose series (if series started, must be completed within 6 months
    • Positive antibody titer (lab report required)
  • Tetanus, Diphtheria, & Pertussis (Tdap): There must be documentation of:

    • Tdap (regardless of when previous dose of Td was received)
    • Td booster every 10 years thereafter
  • Influenza: There must be documentation of Flu shot administered during the current flu season

  • CPR Certification: Students must maintain current American Heart Association Healthcare Professional Basic Life Support (BLS) certification. Information is available at: http://cpr.heart.org

  • Current RN License- All graduate students are required to have an active, unrestricted  RN license in the jurisdiction of the clinical placement.

  • Criminal Background Check and Drug Screening: Annual criminal background check and drug screening is required. YSN reserves the right to conduct random drug screening as needed. Students are required to self-report any changes in criminal background between annual screenings. (Refer to YSN policy on Criminal Background Check and Drug Testing.)

  • OSHA Training: Students receive orientation, including the blood-borne pathogen standard training program, to each clinical facility. The clinical facility provides the training in institutional procedures for meeting Occupational Safety and Health Administration (OSHA) Standards (Bloodborne Pathogen Standard (OSHA)-1910 1030 Federal Register) to nursing students and clinical instructors. It is the responsibility of the faculty of the YSN to provide students with information and instructions on standard precautions for blood and body fluids in accordance with applicable CDC Guidelines prior to any exposure to clients.

  • Individual Professional Liability Insurance: Students must provide evidence of current individual professional liability insurance coverage (minimum $1,000,000 each claim / $3,000,000 aggregate).

3. Clinical Policies

COVID-19 Policies

  1. All students must comply with the PPE requirements of the clinical site.  YSN will provide students with enough masks for 1 per clinical day to help facilitate compliance without burdening the clinical site.

  1. Students, faculty, and staff with COVID-19 exposure or diagnosis must follow current CDC guidelines regarding quarantine and masking.

  1. Students and clinical faculty will be provided with clinical affiliate COVID-19 compliance education, policies, and procedures in place to protect the health and safety of individuals in their healthcare setting. Students and clinical faculty must adhere to all policies and procedures set forth by those entities, including COVID-19 vaccination if required. Students who fail to comply with these policies and procedures will be dismissed from the clinical setting. The student will receive an unexcused absence for the clinical day and the YSN Clinical Attendance Policy will be implemented.

  1. Privacy of student health (HIPAA protected) and education (FERPA) information will be maintained throughout the reporting process.

 

Additional Clinical Policies

Scheduling Clinical Hours

The student should schedule clinical practicum hours that are in keeping with the preceptor’s schedule and availability – not the student’s schedule or convenience. Prior to beginning the clinical practicum, students and preceptors need to agree on the days and times that the student will be in the clinical agency.  The student’s personal and work schedules are expected to accommodate participation in the required number of clinical hours specified by the clinical course. Students may not begin clinical practicum hours before the first official day of the semester that the course begins. All required supervised practice hours must be complete by the end of the semester unless the Program Coordinator authorizes an extension.  Students will be required to submit a calendar of planned clinical hours to clinical faculty at the beginning of each semester.  While these hours may change slightly through the semester, this allows students, preceptors, and faculty members to have a plan for the completion of all hours.

Student Attendance on Scheduled Clinical Days

The student must attend the number of clinical practicum hours consistent with the program requirements Students should not assume that should they fail to complete the required number of clinical hours for the term that they will be permitted to make up clinical hours with their preceptor. Approval for extending clinical hours beyond the semester in which the course is taken must be approved by the Program Coordinator. If a student cannot complete the required hours due to an unforeseen event, the student must notify the clinical supervising faculty immediately to determine if the situation warrants an extension of the clinical practicum and under what conditions this will occur.

If a student is to be absent for a scheduled clinical day (due to illness or an emergency), the student should notify the preceptor prior to the beginning of the clinical day. On the first clinical day, students should identify the procedure for contacting the preceptor in case of absence. Additionally, it is the student’s responsibility to notify the clinical supervising faculty and course instructor of the absence.

Students assigned the same preceptor may not attend clinical on the same day and must coordinate their schedules to ensure there is no overlap.  Students who attend clinical on the same day as another student will have their clinical hours for that day invalidated by the clinical faculty.

Habitual tardiness, defined as more than one day per clinical rotation will not be accepted and may result in a lowering of the course grade, failure of the course, and/or dismissal from the MSN-APRN program.

Transportation and Expenses

Clinical experiences will be assigned near where the student lives and works, as available.  Assignments are made according to the learning needs of the student, clinical requirements, and availability of clinical sites.  Students are responsible for their own transportation and expenses associated with clinical experiences.

Professional Attire

Students should dress professionally in a manner similar to their preceptor.  Professional dress or scrubs should be worn depending on the clinical site.   Students should maintain natural colored hair during the clinical rotations.  Students should wear a white lab coat, which is purchased at the student’s own expense.  A KCU student ID badge should be displayed at all times.

Clinical Rotations at Student’s Place of Employment

Clinical rotations may be completed at a student’s place of employment assuming that student’s responsibilities differ from those performed in the current role and are consistent with the role of the advanced practice nurse.

HIPAA Adherence

All students are expected to comply with the requirements of the Health Insurance Portability and Accountability Act (HIPAA) as applied to clinical education.

Failure to adhere to these expectations may result in immediate removal from the clinical settings and additional academic sanctions including course failure and if necessary, dismissal.

Preparation for Clinical Days

The student should prepare for the clinical practicum as recommended by the preceptor, course instructor, clinical supervising faculty, and the Program Coordinator. This preparation includes understanding and meeting course learning objectives, conferring with faculty on areas of weakness that need to be refined and seeking independent learning experiences that will promote self-confidence and competence.

On the first day of the clinical practicum, the preceptor will orient the student to the clinical practice setting, facility policies and procedures, and required safety and learning modules.

Resources for Clinical Practice

Students are encouraged to develop and maintain a system for accessing pertinent clinical support information at each clinical site.  The system developed may include, but is not limited to: use of a smartphone, tablet, text, or online resources.  Students should confirm with all preceptors if electronic devices are allowed prior to the rotations.

Students should have access to prescribing and diagnostic testing information at all times while in the clinical setting.

Clinical Logs

Students are required to keep a log of all patient encounters and clinical hours throughout their FNP practicum. Students maintain a clinical log in an electronic format. For this purpose, YSN utilizes Typhon Clinical Tracking Systems. The use of Typhon enables students to track the number of patient encounters, procedures, diagnoses and ICD codes, diagnostic testing ordered, and medications prescribed. Students must enter their clinical data into Typhon no later than 10 days after each clinical day.  Typhon will not disclose any patient identifiers.

Student Health: Illness/Injury During Clinical

1. In the event of a medical emergency, hazardous materials exposure, needle stick or sharp object injury, or other clinical-related injury as defined by the clinical preceptor, the student should be seen in the agency’s emergency room, or if that is not available, sent to the nearest emergency room. The student will be responsible for any expenses incurred for these events. The preceptor and student will immediately notify the course instructor and clinical supervising faculty of any such events.

2. In non-emergency situations, the student may verbally tell the course instructor and clinical supervising faculty that they elect to seek care from a private health care provider/clinic. Any cost incurred will be the responsibility of the student. The student and/or preceptor will notify the course instructor and clinical supervising faculty of these events as soon as possible.

3. Documentation of an injury requires the completion of the Student Injury and Incident Report (see Appendix B). The student is responsible for completing the form and obtaining comments from the preceptor or agency representative. The student is responsible for forwarding the completed form to the clinical supervising faculty as soon as possible following the incident. The course instructor and Program Coordinator will review the information and make appropriate recommendations. The document will be filed in the student’s personal file in Student Services.
